NOTICE TO CONTRACTORS
Notice is hereby given that sealed bids will be received in the Columbia County Managers office until 2:00 P.M. on May 3, 2018, for Columbia County Project No. 2018-03. This office is located on the second floor of the Courthouse Annex at 135 Hernando Avenue, Room 203 Lake City FL 32055.
This project includes widening existing 20' paved road to 22' wide for approximately 17,332 LF, or 3.283 miles. The limits of the project are from CR 240 to SW Dyal Avenue. Project consists of roadway and drainage improvements. The existing asphalt will be milled and hauled to the County's stockpile located at 607 NW Quinten Street, Lake City, FL 32055. Limerock base will be thickened, widened, then resurfaced with new asphalt pavement in two layers (structural and surface).
Scope of work includes storm pipe, milling, limerock, grading, asphaltic concrete pavement, erosion control, grassing, painted pavement markings, concrete box culvert construction, and incidental items.
The Bid Forms and Construction specifications may be obtained from the County's web site at http://www.columbiacountyfla.com/PurchasingBids.asp. Deadline for questions regarding construction plans, specifications, and/or bid documents must be received before 2:00 P.M. on May 1, 2018.
The successful bidder will be required to furnish the County Manager with a bid bond, performance bond, and proof of liability insurance prior to commencing work.
The Columbia County Commission reserves the right to reject any or all bids and to add to the contract or delete from the contract to stay within their funding capabilities.

NOTICE OF CONSENT ORDER
The Department of Environmental Protection gives notice of agency action of entering into a Consent Order with the City of Lake City pursuant to section 120.57(4), Florida Statues. The Consent Order addresses the unauthorized discharges to ground and/or surface waters at the City of Lake City collection system on and around September 11, 2017, due to Hurricane Irma and the resulting weather events. The Consent Order is available for public inspection during normal business hours, 8:00 a.m. to 5:00 p.m., Monday through Friday, except legal holidays, at the Department of Environmental Protection, 8800 Baymeadows Way West, Suite 100, Jacksonville Florida, 32256.
Persons who are not parties to this Consent Order, but whose substantial interests are affected by it, have right to petition for an administrative hearing under sections 120.569 and 120.57, Florida Statutes. Because the administrative hearing process is designed to formulate final agency action, the filing of a petition concerning this Consent Order means that the Departments final action may be different from the position it has taken in the Consent Order.
The petition for administrative hearing must contain all of the following information:
a) The OGC Number assigned to this Consent Order;
b) The name, address, and telephone number of each petitioner; the name, address, and telephone number of the petitioners representative, if any, which shall be the address for service purposes during the course of the proceeding;
c) An explanation of how the petitioners substantial interests will be affected by the Consent Order;
d) A statement of when and how the petitioner received notice of the Consent Order;
e) Either a statement of all material facts disputed by the petitioner or a statement that the petitioner does not dispute any material facts;

A copy of the petition must also be mailed at the time of filing to the District Office at Northeast District, 8800 Baymeadows Way West, Suite 100, Jacksonville, Florida 32256. Failure to file a petition within the 21-day period constitutes a persons waiver of the right to request an administrative hearing and to participate as a party to this proceeding under sections 120.569 and 120.57, Florida Statutes. Before the deadline for filing a petition, a person whose substantial interest are affected by this Consent Order may choose to pursue mediation as an alternative remedy under section 120.573, Florida Statutes.
